This study examines the management and prognosis of primary retroperitoneal liposarcomas (RPLS) in an Asian population, involving 21 patients treated between 2019 and 2022 at Gouri Devi Institute of Medical Sciences & Hospital. The research focuses on clinicopathological characteristics, surgical outcomes, and survival rates. The median tumor burden was 36 cm, with the majority of tumors exceeding 20 cm. Surgical resection, often requiring contiguous organ removal, was the primary treatment. The study found a median overall survival of 52 months and a median disease-free interval of 19 months. Factors such as age, gender, tumor size, and positive surgical margins were analyzed for their impact on survival. Despite aggressive surgery, the 5-year survival rate was 49%, highlighting the challenges of RPLS treatment. The findings underscore the importance of complete surgical resection and suggest the need for further research into targeted therapies and molecular alterations to improve outcomes
